The Whitmore Lake Health Hub (WLHH) is a collaborative mission between the University of Michigan School of Nursing (U-M Nursing), Hamilton Community Health Network (HCHN), and Whitmore Lake Area Human Services (WLAHS). This mission is funded by the University of Michigan Health’s Community Health Services through its Community Health Needs Assessment, which tackles various social determinants of health.
U-M Nursing, HCHN, and WLAHS have been working to increase access to healthcare and improve health equity in the small, rural Michigan community of Whitmore Lake. Beyond clinical care, WLHH connects residents to the services they need — including expanded food pantry offerings and other essential community supports.
